Since 2003, Community Housing Sonoma County has been providing permanent supportive housing solutions for people living with disabilities in our community.

We currently have projects in Santa Rosa, Guerneville and Cloverdale. Our projects include new construction developments, and acquisition/rehabilitation of existing facilities. We work closely with service providers to ensure their capacity to own and operate the housing we develop. Key to our success has been our emphasis on developing strategic partnerships with developers and service providers.

In the Guerneville -Russian River Community, we have 48 units of multifamily housing currently under development (see rendering). Thanks to Burbank Housing Development Corporation, this project will provide services for Mental Health clients, persons living with HIV/AIDS, and others. Dwelling units - townhouses and stacked flats - will be raised well above ground level in this flood zone, with their entrances facing a high, specially built 'street'. The project also includes a community center with management offices, laundry, and a commons room.

Hearn Avenue ProjectWest Hearn Avenue Housing (prior to restoration)

On West Hearn Avenue in Southwest Santa Rosa, we are refurbishing a facility to provide transitional housing for armed services veterans. This project will include 12 transitional housing units and future permanent supportive housing through the support of our project partner, Vietnam Veterans of California.

We also are working with service providers in Cloverdale and Santa Rosa on aquisition and rehabilitation, as well as new construction of permanent supportive housing projects. The Sonoma County Continuum of Care program has recently committed funding to the Cloverdale project.

  • Rohnert Park Expressway, Rohnert Park, CA
Community Housing Sonoma County and Burbank Housing Development Corporation have constructed a new, 100% permanent supportive housing complex providing affordable housing and support services to formerly homeless families and individuals living with disabilities. A broad spectrum of services are being provided, including mental health and chemical dependency support, employment readiness, parenting education, and money management. Volunteer teams from the community, trained by The Family Connection program, will be giving additional support to the families residing at Vida Nueva.

Vida Nueva
  • Project: Larkfield Oaks
  • Fulton Road, Santa Rosa, CA
In partnership with Burbank Housing Development Corporation, we have created this multi-family rental development for low-income singles, families, people living with disabilities, and agricultural workers. Through Community Housing Sonoma County’s leadership, seven units are set aside as permanent supportive housing for persons with mental and/or developmental disabilities. Additionally, the special needs residents have vocational employment opportunities directly across the street in Sonoma County’s first open space “urban farm” on a publicly designated open space property.

Larkfield Oaks

  • Brown Street, Santa Rosa, CA
Community Housing Sonoma County, as turn key developer, provided the financial expertise and well-established relationships with service providers and local funders to ensure the preservation of this facility for the fifteen women residents who live there. Community Support Network provides full time services that are designed to meet each resident’s needs. The program is intended to empower residents to maintain stability within a community setting while striving toward maximum independent living.

Brown Street Residence
  • Project: Stony Point Commons
  • Stony Point Road, Santa Rosa, CA
Community Housing Sonoma County partnered with Burbank Housing Development Corporation in the acquisition and rehabilitation of this facility for very low-income residents living with mental illness. Each tenant has their own room (SRO unit) with individual lock and key, appliances (microwave and refrigerator), and access to shared community spaces, a large living room, dining room, and a community kitchen. Residents participate in weekly “house meetings” and are asked to do one hour of yard work per week on the beautiful grounds of this property.

Stony Point Commons

  • Moorland Avenue, Santa Rosa, CA
With collaboration from Burbank Housing Development Corporation in the development and ownership of this new multifamily, affordable housing development, we provided permanent supportive housing options to disabled individuals and low-income families through a set aside of units. Community Housing Sonoma County supported and helped to coordinate Burbank Housing’s efforts with Buckelew Programs to target five units at Carrillo Place for people living mental illness; and with North Bay Housing Coalition to target five units for people with developmental disabilities.
